My experience is its the 1911 shooter used to keeping their thumb on the safety that have this issue -- I used to be one. The 1911 is still my favorite to shoot, but because I don't carry a 1911 I still have the habit of wiping off a safety on the draw even if its not there and broke my habit of riding the thumb safety when shooting.
I ride the thumb safety when shooting, and most people I see shoot my USPc do the same since otherwise the safety is kind of in the way. The only way I can do it is if I'm consciously trying to force the lever all the way down.
 
Riding the thumb safety is less of an issue with the USP than the others with this system, but only the Beretta 9000S really solved the issue by making decock be up past safe.

There also is a difference between gripping with the thumb on the safety vs. actively using the lever for leverage during recoil -- this is where you get the accidental decocking.
 
I bought one of the first USP's imported into the country. I still have it, and have put about 10K rounds thru it in the last 20 years. They had a recall to change to a captive guiderod spring....but I chose to leave mine as built. It also has a conventional rifling barrel, as opposed to the polygonal installed quite soon afterwards. The only hiccups it's had, was with crappy ammo....and that was probably less than a handful of times.

My HK pistol collection has evolved[backwards in time], and I have mostly P9 series pistols now, and a few rare others.
