No 'one solution' works for everyone but this is what works for me. I almost never use an OWB for carry except sometimes when I wear a suit jacket (sundays sometimes). In that case I have an Alessi belt slide that really holds the gun nice and tight - 3:30 / 4:00 position.
I usually carry my .38 snub in my right front pocket or appendix carry IWB at about 2:00-2:30. When I'm carrying my Commander it is usually at about 3:30-4:00 or crossdraw at 10:30-11:00 when I'm driving. I find the appendix and crossdraw positions much more comfortable and accessible when seated, as when driving. I have no problem concealing any reasonably sized sidearm in those positions either, as long as I pay attention to wardrobe. However, since I live in an open carry state, "printing" or "flashing" is not really a big deal anyway, and I do open carry on occasion, mostly out in the woods.
