Home made SBR - what model for Form 1?

I am turning an Uzi parts kit into an SBR. The Form 1 asks for a model. Would I call mine an Uzi as far as the paperwork is concerned? I would think it would be trademarked, like AR-15 is. Can I call it whatever I want?
 
Who made the receiver? That is what goes on the form 1 for the manufacturer. Is there a model number stamped on it?

For my "MP-5" SBR, for the form 1, the manufacture (4a) is Vector Arms, 270 West 500 North, North Salt Lake UT and the model (4d) is V94. No MP-5 anywhere. You go by what is stamped on it.
 
I made the receiver. Uzi receiver stubs that were torched through the original data welded to a center section. Basically, scrap metal turned into a gun through the magic of electricity.
 
Then you are the mfg and you can call it whatever you want for the model. IANAL but unless you're marketing it, I don't think you have to worry about trademarks. But you may want to come up with your own. Though now you have more to engrave than just your (or your trust's) name and location. You all have to assign it a serial number. Read paragraph 2i of the form 1 instructions.
