How concerning would this be to you

Today while getting some rounds through my new CZ P-01 Omega on about the second clip the gun would suddenly not fire. Hard stop on the trigger just prior to break point. Upon inspection the decocking lever had walked out just a smidge. Essentially the amount required for the first step if you were removing it for conversion to a safety.

I snapped it back in place and fired another 140 rounds or so without issue. Hard for me to understand how it would have maybe not been fully seated to start. I’ve had it apart once just to learn the conversion but had shot a few hundred rounds since then.

I inspected it closely during cleaning and everything is correct and in place.

Fluke to keep an eye on or don’t carry until it proves itself?

Should add it’s new in the sense that I’ve only got about 500 rounds through it. Have had it a few months now and the only other thing done is that I had my local shop change the sights out.

Edit: Basically whats shown in the picture, which cant be done without depressing the ejector, which cant be done with the slide one so I had to of bumped it during the last cleaning.

If I could prove to myself that it couldn't happen unless the gun was first messed with while the slide was off, then I wouldn't be concerned about it happening again.

It would bother me tremendously that I did something to the gun that caused it to stop working properly, especially if the gun in question was intended for self-defense. That would be telling me that I was either too careless when working on the gun, or that I wasn't competent to do whatever it was I tried to do. I would definitely work very hard to figure out how to solve those two problems since the outcome could potentially be life-threatening.
 
The pic shows how it was after the ftf. If it had been like that prior:

1. it would have failed to fire on the first go.
2. I would hope I’d notice that.

I can not physically get it into this condition with the slide on so in guessing it was in some intermediate position that I’ll need to try and reproduce. Other than that I’ll go pound some rounds through it.
 
First I would disassemble the safety again, check everything and repeat the reassembly procedure, taking care to follow the instructions to the letter and watching some tutorial videos to be sure that everything is done correctly.
In my opinion, the end of some wire spring that should hold everything in place was not fitted correctly.
In any case I would put the pistol out of the defense gun rotation until I was 100% sure that the problem is solved.

Spring end is in the hole and under the tab as it should be. I did take it apart and reassemble for good measure. Tried to duplicate by trying to push it our while cycling the action in case it is somehow engaging with the ejector. Could not reproduce failure. I’ll just go shoot a bunch of rounds and see

If you really want a nice trigger, CGW or CZC are options. If you're a DIY type, you could go that route. Decockers are a little tricky but do-able.
Lastly, you might consider a conversion to a manual safety if the Omega models allow it.

I agree if the failure wasn't caused by the gun coming apart. CZ sear cage work is very precise, hope it was just an assembly error.
