I'm not sure how those of you advocating a punt to the head would kick a standing man in the head... unless you are Jean Claude Van Dame. A head kick is not practical and is unlikely to knock the guy out. At best he may be stunned for a moment and then enraged he charges you with the knife. There is a good chance that you will be off balance after your soccer-style kick and may even fall. At this point you are at VERY close range in a VERY disadvantageous position. Not a good idea.

Unless you are wearing steel-tipped boots a kick to the head is very low yield and likely to throw you off balance.

It is also risky to try to sneak up on a situation like this. You never know when he may turn his head just enough to catch you in his peripheral vision or the girl may alert him to your presence with her eyes or words. Another bad idea.

Keep a safe distance and verbally challenge the guy. Reassess and base your strategy on his reaction.
 
You don't need a conventional weapon when you are against a wall in a fight for your life. There are plenty of creative ways to utilize car keys or an ink pen. If you are desperate use anything you can get your hands on. Luckily I've never had to use them but I've deployed my car keys in a clenched fist on a few occasions when I expected an uneven altercation. They would likely be lost after a few blows but a few key shots in the face and neck will do in a pinch. This would likely be construed as deadly force but if you are in a fight for your life then that's the least of your problems.
