Round under extractor

I do not have this problem. I am transitioning from moon clips to speed loaders. I know speed loaders prevent this failure. Without the moon clips, how do I avoid this problem?
 
Muzzle up, punch the rod down smartly to eject empties.

That also dumps the unburned powder out on the ground instead of it getting it stuck under the ejector.

What the others said: Get the muzzle vertical and eject with authority.

And one other tip: If you get the charge holes chamfered, do not chamfer the portion that comprises the ejector - that'll greatly increase the chance of a slipped case.

Under normal conditions, and using proper technique, a round slipping under the ejector is exceedingly rare. If it does happen, though, push a thin pen or pencil into the other end of the cylinder to help push the case out. Trying to extract the case with your fingers will take a lot of time.
 
Yup, muzzle up, cylinder open, I like to put my strong them on the hammer spur (if it's got on) to make sure it's clear of the brass. Hit the ejector hard, bottom it out, and that brass will go a flyin!

It really is absolutely not an issue with a quality revolver. I have NEVER had it happen while shooting or during reload drills. IMHO, one is more likely to get a bound revolver from a bend moon clip than one is to get a rim under the star when not using mooners.
