Stevens 58 failure to feed

I picked up a Stevens 58 12-gauge 2-3/4" with magazine at auction. With either live ammunition or with snap caps, it will not reliably feed rounds from the magazine. It's quicker to leave the magazine in (prevents dropping ammunition on the ground... very embarrassing) and simply use the gun as a single-shot. I disassembled the magazine and tried all combinations of reassembly (only three pieces; the case, spring and lift plate), with no improvement in failure to feed. Has anyone else had this problem? Fixed it? How?
I'm also looking for an adjustable choke for the 58, but Numrich does not have all the parts and pieces. Anyone have a lead on one?
I have a schematic for the shotgun on the way from Ken Rosenow at "www.radioreprints.com", but does anyone have a user manual I could pay to copy? Yeah, it's a pretty simple weapon, but I like documentation...
