SX3 burst barrel, anyone see this before?

I had an 870 blow the choke tube and end of the barrel off a few years ago while duck hunting. IIRC, first shot of the day, I pulled up on a drake and pulled the trigger. To my surprise it didn't drop, and when I lined up again for a second shot, there was something sticking up off of my barrel. It was the vent rib.
At first I thought I must have got mud in the barrel, but I was in my boat. That made no sense. It was a pretty new 870, so I called Remington and sent them the barrel. A month or so later,I got a new barrel, and an explanation that the choke tube threads had been machined off center.
I thought Remington CS did fine for me.
 
Update. Winchester ammo contacted Winchester firearms and had them send us a new barrel. I asked for the technical failure analysis report, but it turns out that they never really looked at it. Anyway, hats off to Winchester ammo support.
 
I've seen the same thing happen more than once. Most were caused by a barrel obstructions, mud. But one was at the T&S range, blooper.
