Two quick questions about UT non-res permit

Two quick questions on the Utah nonresident application-

1. Have you ever been convicted of an offense involving the use of alcohol?

Seven years ago when I was eighteen I was arrested for under age consumption of alcohol. The form says "attach documentation explaining your answer". Do I just need to sum up what happened or do I need the actual ticket? Does this hurt my chance of getting the non-res permit?

2. The form has a space to be filled out by an Utah CCW certified instructor. Is there a way to skirt this requirement because I am active duty Army?

BTW I have my AZ resident permit and I am currently stationed in colorado.
 
Answer yes, attach a brief description that it was 7 years ago. Utah has a 6 year look-back. This will NOT be a problem as you describe it.

If you call the Utah folks you will get excellent service and advice. The people that work in that arena are very helpful. Not an issue, in general, if over 6 years ago.

It is a great Permit to get!

Thanks for the replies and links. I didn't think the alcohol incident would be a big problem but I wanted to ask anyways. To get your Colorado CCW you need to take a CCW course or be active duty military. I was hoping to get lucky with Utah too, but it won't hurt to take another course.
