What might cause a failure to extract (CD HP)

I'm returning my Charles Daly HP to the factory for repair. It experiences failures to extract at least one out of ten rounds. The case is found remaining in the chamber with a live round crammed against the case head.

I contacted Charles Daly and was informed I would receive a shipping label for free shipping to the repair facility... can't get much better customer service than that. However, I am curious what the culprit might be.

The failures cannot be replicated by hand-cycling the slide, though the cartridges do "fall" out of the ejection port rather than being slung out as typical with my other autos. Is this common with the hi-power?
The chamber is smooth and after inspecting the offending shells retrieved from the chamber, there appears no damage to the rim... no marks at all.

Out of the approximately 150 rounds I put through it, I did experience 3 "stovepipe" failures to eject as well. Feeding was flawless with all rounds (assuming there were wasn't a spent case in the chamber, of course)

Does anyone care to take a guess at what might be causing these malfunctions?
