What model 12 do I have?

That looks like 70s or 80s era Fajen wood.

Im hesitant to say......but
An unaltered mod12 30" full bbl, Fajen stock and fore end.......$800 to your brother, $1000.00+ for anyone else.

When I pulled/set trap/skeet in Nebraska (69-70) I saw (and drooled over) many model 12 trap guns like that, especially with the wood pistol grip cap and the high/wide Simmons rib.

Correct the field grade has no marks right there

I think that may be a timing mark so the barrel is oriented to the receiver.
