Can't give you the model, but an easy way to determine if there's real steel in the barrels is to dismount the forend and look there. Usually the surface is in better shape there. If you see swirls or grain, it's twist or Damascus.
Please have a competent smith go over this and all other old shotguns before shooting. Grenades and shotguns have similar working pressures....
