There is an interesting article on The Manhattan Conversion for Colt replica cap 'n ball revolvers. The conversion entails brazing/soldering a piece of metal to the frame that prevent spent caps from jamming between the frame and the hammer. The hammer must be modified by slimming on both sides to allow it to clear the metal insert.

The modified hammer looks somewhat like a Ruger Old Army hammer. Imagine the frame like that of the Ruger but with an open top.
 
I think the old cowboy movies where the shooters "threw" their guns down as the fired each time was a work around to the jamming cap phenomenon, also.
