I suspect the biggest problem is with rough chambers (from marine coat, dull chrome, applied coatings, etc), tight tolerances and the steel/aluminum head "bargain" shells.
some of the cheap Winchester low base shells visibly expand and like to stick in my chamber sometimes. i dissected a few of the culprit rounds and noticed the plastic hull was noticeably thinner in those, causing them to swell.
