I mentioned previously on the same subject but differnet post tha the pictures of breach face looks like it might be slightly too narrow, jamming the rim when it starts up under extractor. The rim slides under extractor and is then held captive not forced under which would have been disclosed when extrator was removed.
A little polishing on breach face and filing the corners with jewlers file until rim slides right up under extractor. I suggested this before but several experts really peed on my shoe's about it . I'm a novice I've only built about 250 Government models since I started and this shouldn't be that bad as it seems, everything in the world has been mentioned twice at least except too narrow breech face. Every single caliber slide has a different width breach face and one can be slightly too narrow In the first few pictures there is brass smears all way to the corners on breach face. If the sheel is not being push straight it can't go straight. Try it.

Looking at several of those rounds, I would suggest giving your seated rounds a "kiss" more crimp. I know that the 45 ACP head spaces on the case mouth, but sometimes too much case mouth expansion and not enough of a taper crimp will cause the same FTF. I have seen this happen on three different occasions when I was shooting Bullsye Matches.

45ACP: The most simple way to correct feed problems including feed and fail to chamber is.......

Had this problem with one pistol, it was recommended I send it back to Ruger, not easy to take a chance a repair will alter accuracy, I choose accuracy, the pistol liked store bought ammo, new stuff, all the problems happened with reloads, so I examined ammo and compared it with reloads, some of my reloads looked like cases that had swallowed a bullet, if it can be seen, it can be measured, so I started out to duplicate new ammo appearance with my reloads, after failures, success, after loading seating and crimping I installed the carbide full length sizer in the press and crimped the cases with the full length sizer 1/3 down the case. the bulge was removed mouth of the case blended into the bullet, this left nothing to hand up on and allowed the case to chamber, my pistol thought the cases were store bought.
