Take the dang bent mount off, and shoot the dang rifle with open sights!
That will tell you right there if it is the rifle, the scope set-up, or the guy doing the shooting.
If it groups with open sights, and you can shoot it, then you have a scope & mount problem.
And from the looks of the warped mount, and what appears to be the scope touching in two places it shouldn't be, I'd say you do have a scope & mount problem.
