What is your worst gun accident?

"Your probably wondering, did I shoot five times or six?"

A friend and I were shooting junk in the woods with our single-six Rugers. We both were saftey concious and mentally counted our shots as most probably do. I fired my sixth round then pulled one more time to be sure and the gun went off!. We looked at each other in amazement and decided that was a sign and quit for the day.
 
Not buying one of the bargain bin AR-15's from CMMG while they still had them. Also had an accidental double tap one of the first times I shot a pistol. Still had it pointed downrange at the target though.
 
Had my old Steven 22./410. combo and as I fired at a squirrel I saw the end of the 410. barrel went flying. I don't know what caused that 3" chunk to fly but it scared the crap out of me.
 
Wore a blister into my thumb after around 85 rounds through a J-Frame yesterday.

Edit: To clarify why it was an accident: I spent twice as long at the range as I meant to.
 
I also had an experience similar to UTDave's. I keep a loaded mag in my .40 and my brother was down for the weekend. He was fondeling my guns while I was playing videogames. I looked over and saw he had my handgun. Before I could say anything he racked the slide and started pointing it at the wall. I told him that he had just chambered a round and that pulling the trigger didn't seem like a very good idea. After he unloaded it he told me I shouldn't keep a loaded gun. I told him that an unloaded gun was just a crappy club and told him that it was actually him that loaded it since he racked the slide. Anyway, now whenever someone wants to look at my guns I will make sure they ask me first so I can unload it.
 
The worst part? Nothing happened. No neighbours noticed, no cops, no investigation by apt manangement that or the next day. I live in a big complex with apts closely grouped together too!!!

Yeah, my worst accident was also a dumb discharge, right through my bathroom mirror from my CZ 97B, I think it was 230 grain WWB JHP, I lived downtown, it was on memorial day, of all times, police station across the parking lot from my apartment, fairly high population density in that neighborhood, but not one person noticed, there was no ivestigation, nothing. It was the most embarassing thing I've ever done.
 
Was at the range and knocked my fairly new stainless Ed Brown Executive Carry off the counter and on to the cement floor. Thankfully no apparent damage (other than my heart).
