Where to get FD-258 Fingerprint Card for Utah Permit

I'm putting together the applications for my Utah and Florida non-resident permits. I already have the training requirements met, and I have a Utah application stamped by a Utah-approved instructor. I'm going to get my prints taken at my local police station next week, but I've hit a snag: they don't provide FD-258 fingerprint cards.

Florida was kind enough to include an FD-258 in the application packet they mailed me, but for Utah all I got was the application form itself, from my instructor. I've looked around and my town and county PD both require you bring your own cards, I haven't found any private fingerprinting services locally, and as far as I can tell you can only buy FD-258s in 100 or 250 count packs.

Has anyone else had this problem? Where did you end up getting the card?

pretty sure you can order them from the florida website that you order the rest of your forms for ccw....they ask how many you want, i get extras to avoid hassle if it gets screwed up.

try that.
 
FD-258- that's the FBI fingerprint card, #258, correct? If it is, then ATF has them available on their website, you can order as many or few as you wish- shiopped to your door for FREE. However, mine came with NFA Branch, Martinsburg, WV routing information on them. I actually have a couple laying around, and I would gladly send you 2, but they aren't marked properly for Utah BCI.
 
Thanks for the responses. I was able to order plenty more from the Florida CWP site; I never even noticed the option to order the cards separately.

I've read elsewhere that whited-out routing information is pretty common on these cards, so for the Utah prints I'll just white-out the Florida info and let the Utah folks fill it in.
