I bought a Kel-Tec P11 years ago right after they first hit the market. It was reliable but was not much fun to shoot. Shooting one or two magazines was about my limit before the web between my thumb and index finger hurt too much to continue. So, despite its fairly small size I never carried it much. Now my concealed carry holster gun is an Astra A75 in 40 S&W. It's a heavy piece of steel but fairly compact, accurate, and shoots well. A month or so ago I shot a friend's P3AT and had to have one. With a Hogue Handall Jr and the plastic mag extension, it is very comfortable for me to shoot. I have had no misfires or jams in almost 400 rounds. I did the mod to the feed ramp to eliminate the 'smiley' problem on the nose of the bullets. It was not necessary for feeding reliability, however. Accuracy is excellent at 7 to 10 yards. This little mouse gun goes everywhere with me. I especially appreciate it when I am going or what I am wearing makes the Astra not work....
 
Another vote for the Makarov: Inexpensive (but not cheap), very accurate/reliable, ammo's cheap so you can afford to practice a lot (remember, when it comes to self defense, only hits count...), good SD ammo's available (Hornady XTP, Georgia Arms Gold Dot load, etc), recoil is mild (especially w/19# or 21# recoil springs) and has a number of concealable carry options.
