This is a purely theoretical question for those who have a solid understanding of gun mechanics. It is in regards to at least 2 posts I have read about carrying one in the chamber or not.

This is not something I would ever do in real-life. It's a theoretical scenario.

A Beretta with a safety on chamber loaded. You pull the trigger. Is there any way, if every internal and external safety mechanism failed, that a discharge would occur?

The reason I like the external safety is it prevents the "the trigger got caught on X and discharged" excuse. I do not rely on the external safety, it's just additional insurance. Like all insurance, it comes at a price. The premium paid is that I must remember to disengage the safety.

This is not a debate about whether to have safeties or not. It is not because I want to ignore proper safety. It is a worst case, Murphy's law scenario. It is a simple question, is it physically possible if all mechanical safeties are broken, for a Beretta to discharge with the safety on and the trigger pulled?

I have been eyeing a Glock or XD for a while now, but I want to give my Beretta a fair hearing before trying something new.
 
For the sake of discussion, I assume we are talking about a 92FS or a PX-4 F with the safety that disconnects the trigger when on.

I own and have owned a lot of Beretta duty pistols. Here's my take:

Yes, theoretically, a Beretta with totally failed safeties would fire if one intentionally pulled the trigger.

The trouble is seeing how that could possibly happen. The safety lever would have to rotate down with two different things entirely failing. First, the trigger bar would have had to have somehow not disengaged with the rotation of the safety lever. Secondly, the aft part of the firing pin assembly would had to have not rotated out of the way of the hammer strike.

I have never seen a Beretta in such condition. Surely a totally floppy safety lever would give away its broken condition to the user and force the pistol to be put in for repairs.
