Download the 4473 form in PDF format from TitleII.com, check the appropriate boxes / play fill in the blanks, print on yellow paper, and you're good to go.

When it came up on my screen it was partially filled out. Is this a legit form to buy with or just a sample with instructional comments. I kept it in my favorites for future reference though.

I believe that it isn't partially filled out; you can type in the correct information on your computer before you print out the form. Such as where it says "Name", you highlight that area and fill in the correct name, etc. When you print it out, all the correct information will be on the form in nice clear print.
 
I'm using this BOS for my guns and other personal property as part of my will/estate planning.

Does anyone know if a BOS is completely legal without being notarized?

JMiller, 444's got it right... once downloaded, a PDF reader (in my case, Acrobat 6.0) will allow you to change the checkboxes and fill in the blanks on the screen. The older 5.0 version just showed the plain document. You could print it blank if you wanted to, but it's intended to let you print out a copy with nice clear print.
