Change in address on Form 1

I need to fix a mistake on a pending Form 1. After triple checking everything on the form I still missed a blatant mistake and didn't catch it until after I had mailed off the form and it is now pending but not approved. I've left a message with the ATF regarding my question but haven't received a call back so thought maybe you guys could steer me to an answer. The mistake I made was combining my old address with my new address. Meaning I have my new street address but combined it with my old city,state,zip. I also put my old city,state in the additional description/engraving portion. Needless to say I will be sending in a letter of correction to change the address to my current address. But can I also include in that letter a change of the engraving information. Also does anyone have a "rough draft" or list of information I need to include in this letter. I can't believe I missed this mistake but need to know what to do to correct it.
