Help with unknown shotgun???

Please help me identify this shotgun. Pictures are attached below. It has very minimal markings.

Top of barrel: London Fine Twist
Rt side of receiver: CCW (Possibly, hard to read) Arms Co Norwich Conn
Serial/Part No: 5603
Barrel Length: 33 1/2"

Any help is greatly appreciated!

My first thought was Crescent based on sheer numbers made, with the name being whatever a hardware store wanted on it, BUT, there are lots of other possibilities given the location. I tried some online searches using CCW, CEM, x-whatever and got nothing.

Maybe someone will recognize that interesting design, I know I'm not familiar with it.

Norwich was a busy place...

"Firearms
Norwich was considered "the firearms center of New England" from the Civil War to the Depression. It is said that in 1860, more firearms were made in Norwich than in any other city during the War Between the States. Manufacturers included: Bacon Arms Co.; Norwich Arms Co.; Norwich Nickel & Brass; CC. Brand; Whaling Gun; Cole & Walker, Machinists; Alvah Grimes, Gunsmith; Osgood Gunworks (also known as Norwich Pistol Co.); Hood Firearms Co.; Thomas E. Ryan, Gunmaker; Davenport Firearms Co.; Crescent Firearms Co.; Manhattan Arms Co.; Ethan Allen and Charles Hurber; Tobin Arms Co.; Hopkins & Allen. The most recognizable name in Norwich gun manufacturing was Smith & Wesson, which began on Central Wharf in Norwich, later moving to New Haven when the popularity of patented revolvers required a larger facility. The last of the guns and ammunition manufacturers closed in 1931, but the Guns of Norwich Historical Society is helping to preserve that part of the City's heritage."
 
Look at what I found, not that it proves anything really.

"In 1888, George W. Cilley bought out the defunct Bacon Arms Co. of Norwich, CT. He then formed an alliance with Frank Foster, and borrowed enough money to form the Crescent Fire Arms Company. Cilley and Foster each held several firearms patents, and both were highly qualified in firearms design and manufacture. Production began with single shot tip-up shotguns that had an external side hammer. Double barrel shotgun production was started in 1891. "
 
Does anyone know where I can get a replacement part as shown below and what it is called?
