i started the OP with descriptions of several actual events in which firearms that were believed to have been unloaded were fired with no tragic results. I added a scarier story later.

The timing coincided with the story of the tragedy that occurred on the set on which Rust was being filmed. A number of posts have alluded to rumors, and to facts that have been made public, about that case.

There has been a lot of speculation and conjecture about potential liability in the case. That is never a good idea, and we usually close threads that venture far into that area.. The only thing that appears highly likely at this point is that should Baldwin be charged, the facts indicate that Baldwin should be convicted of involuntary manslaughter. That would not rule out the possibility of other charges, or of civil liability, when more facts have become available.

We have learned that the ammunition definitions used in the entertainment industry differ from those used at gun ranges, etc.

We have also been provided with a set of the comprehensive safety rules and procedures used in the industry. Based on my experience in risk management and in the design and testing of systems of internal controls in the corporate world, I am almost certain that attorneys and other experts will soon be re-evaluating those rules and procedures for adequacy based on the Rust tragedy, if they haven't started already. You can bet that training requirements will not be overlooked.

I am closing this thread after two hundred posts. We can discuss the Rust case later as more events unfold.
