Yes it happens if not safe. Most have had a ND. And most put a hole in the floor or in the dirt. Not into a friend. Does it happen absolutely. Why does it happen? Not following the rules.
 
Earlier this week the leader of the NFAC posted up a video announcing the march.
He laid out the uniform requirements, equipment requirements, etc.
He then went on to invite those meeting said requirements to show up at the protest, where you'd be sworn in.
You had to have fired your gun before. And if you were wearing a plate carrier it had to have plates.
Zero mention of anything safety related, and certainly no requirement to demonstrate such knowledge.
As far as I'm concerned he owns anything that happens there. He won't own it, as evidenced by the quote above (no big deal, right? These things happen!), but he should. And should be held to account.
Apparently that was not the last ND at this march. I've heard they had at least one more.
