The key protruding from the fist is not as effective as it is usually presented since the keys don't want to align and do want to rotate. They're usually a once and done technique to help you surprise your attacker and allow you to escape. Still useful for that purpose, but limited to that.

When my wife's hospital gave the training they didn't have anyone put their keys to use. I showed my wife what actually happens when you punch with a keyring in hand and it was quite a revelation. Good once, but not very effective for repeated use.

My brother in law told me that, to poke someone's eye out would be about as difficult as poking a finger into a baggie full of water.

I suppose, if all rules are off the table and it is for life and death, poke the bad guy's/gal's eye out! Even with one eye, I really doubt they would keep coming unless they are all hopped up on some pain killers.
