It is a modification they made to the extractor to allow case rims to slip under more reliably, however it also compromises the purchase the hook has on the casing during extraction. What they are counting on is the next cartridge/mag lips to keep the case in-line with the ejector to get kicked out the ejection port.
The problem is when the casing comes loose from the hook, it has nothing to pivot around to kick it out sideways...that is what makes the erratic ejection pattern .
Worst case is then the casing kicks sideways and hits the inside of the slide...bouncing back into and tying up the action
