Has anyone configured the factory safety lever so the handgun safety is on in the up position? It has been a while since I have had mine apart and I can't remember if anything would prohibit the mod. Wonder what it would cost to have the safety lever/pin machined?

You can have it cocked and locked.

The slotted bar/pin would have to roll from the bottom.
 
When I had my Mod. 457 lever up was de-cocking position (on some PC guns this lever is spring powered self-returning after being depressed). Initially I tested mechanism using Italian caps making sure pin did not fall on the primer. In my gun it didn't the hammer just went up and gun could be carried with one in the chamber to be fired in DA mode for first shot.

You sure about that?

I have never seen a S&W in my life that worked like that.

Reason being, the slide slamming shut would be trying to flip the lever straight foreword and decock the gun each shot.

Down has been decock and Up has been fire on S&W's since they copied the Walther P-38 safety design in 1950 something.

It was rather strange in that it had spurless hammer yet it was not DAO. I traded it for Model 12-2 but remember that lever worked as de-cocker just like in PPK and Makarov. Unlike in Makarov however the lever in horizontal position did not lock the slide.

Not on my 457 that I've owned for ten years. Down for de-cocking the bobbed hammer and placing on safe. Leave it horizontal for SA first fire.

Whatever I did with the lever the gun was hot hammer down and it would go off when the trigger was pulled. I can't test it now because I send it down the road about 6 months ago.

All DA's of any brand are hot with the hammer down and the safety off.
That's what the Double-Action feature does.

BTW: No S&W 39/59 series safety/decocker locks the slide.
