Seems unlikely that the exposure you described would cause permenant hearing loss unless there was already an underlying ear disease. Was she evaluated by an audiologist as well as the ENT?
One thing to be cautious about with new shooters is that the muffs you have may not fit everyone as well as they fit you.
My wife has a very hard time finding muffs that seal properly behind her ears and generally wears plugs and muffs together as a result. I did'nt realize the muffs weren't working/sealing right until she complained about her ears hurting after a shooting session at an indoor range and we started investigating.
New shooters don't realize how well the muffs should work and may be too distracted by other things to pay a lot of attention to the noise.
