What does it take for you to consider a pistol unreliable?

1/1000 has been my idea also, for a gun that may be used to anything a bit more serious.
But depending on the gun type, one probably also has to consider the overall round count. Some guns can only take so many rounds before they are ready for the trash bin, or a serious rebuilding is in order. For example, a friend shot circa 20K rounds (+P) through a SigSauer P226, and the frame had been cracked in 3 places. That particular pistol had effecticely become scrap metal.
 
A pistol isn't reliable if it's fired 2500 flawless FMJ's and burp's on the first box of hollowpoints. I'll put at least 500 rds of premium(HP) ammo before i even consider it for serious use.
 
For me, a pistol that is 99% reliable is good enough for plinking and such. For a carry gun I demand 100% reliability. I have a Glock 21 and a Russian Makarov that both meet this criteria. Despite what you read in the gun rags, clearing a jam isn't an option in a gunfight. In a 2 second gunfight, being able to clear a jam in 3 seconds is just too long

My CZ P-01 would jam 2 to 3 times per magazine at worst, 1 time per magazine at best. I sent it back to CZ and when it was returned, it would jam 3 to 4 times in 50 rounds. It did not make the grade. Its replacement, the Stoeger Cougar, has been perfect in every way.

My HK's have been perfect.

My Kimber has been perfect after about ~300 rounds.
 
I haven't had a single malfunction out of my P239 SAS for 4000+ rounds. I trust this gun with my life. This is how all guns should function.

On my P3AT however, I know it will start to have FTE due to dirt/grime when you get closer to that 100 round mark. Once I clean it, it functions fine. To me this is acceptable because it's a BUG gun and I don't carry more than a spare magazine. Out of a primary weapon, this would be unacceptable. I have however had one light primer strike out of this gun.

A Kimber pro carry I fired was very accurate however that thing would have one FTE every other magazine. Now that would be totally unacceptable to me.

I guess it really boils down to whether or not I can figure out what is causing the malfunction and fix it. So I guess my answer is: it depends? Once I get frustrated with constant malfunctions is usually when I call the gun a piece of crap.
