I didn't know there was a stigma over cocked and locked. In fact, if you own a Ruger or Browning .22, you carry it cocked and locked -- the hammer is internal and you can't see it.
The most common cause of "Glock butt" is getting a fold of clothing shoved into the holster with the gun -- easy to do when your attention is focussed on something else (like controlling a suspect) when you holster your gun.I know two officers who have ended up in the hospital from "glock butt."
