Really bad TV and movie gun handling

One of my favorite, "look what those idiots did", scenes was in 24. Jack Bauer is interrogating a suspect, pulls out his H&K, and points it at her head - threatening to shoot her if she doesn't talk. When she doesn't, he racks the slide (you know, to "show that he means business"), and of course, no round pops out. So, what? The first time he was planning on dry-firing her to death?
 
How about the different csi shows where there elbows are bent so much that the slide is 4 to 6" from there nose to get it all in the picture frame.

It's possible he doesn't keep a round chambered and also because he thought he might need to rack the slide...I mean he is Jack Bauer.
 
Charlie's Angels (the original 1970s series) had some pretty poor gun handling, but at the time I wasn't really paying attention. :D So did The A-Team. In the opening credits, Murdock (Dwight Schultz) fumbles with and nearly drops his Ruger AC556. As well, none of their gunfire hits anybody.
 
Or Horatio in CSI: Miami with the half-gangsta style grip....

and when the sound effect of a gun being cocked even when it is not a double action.
